Coordinated Family Services
The mission of Coordinated Family Services is to support individuals in achieving independence by working closely with their families. We help them identify and access available resources to enhance their overall quality of life.
What to expect
Identifying and providing the necessary supports to successfully live in the family home.
Helping the individual and their family learn how to use services and support that fit their specific needs. This includes finding ways to overcome challenges in accessing general and other resources.
Helping to set up and organize medical and other appointments.
Finding transportation options or services.
Community Engagement
Helping the individual plan for the future, including those who have aging parents/caregivers.
